Patients in India already message their doctor's front desk on WhatsApp. The problem is that it usually lands on one receptionist's phone: replies wait until someone is free, reminders get typed by hand, and nobody can see what was promised to whom. The WhatsApp Business API fixes that. This guide covers what clinics, hospitals and diagnostic centres use it for, what each message costs at Meta's current rates, the rules you need to follow, and how to set it up.
The short answer
- Use it for the front desk, not for medicine. Appointment confirmations and reminders, "your report is ready", directions, timings, payments and follow-up visits are what WhatsApp does well.
- It's cheap per patient. Meta charges ₹0.115 per appointment or report update in India, before GST. A confirmation plus a reminder costs about 27 paise per patient with GST.
- Get consent, and keep health details out of messages. Meta requires opt-in, and its policy bars sending health information where the law requires more protected systems. Send "your report is ready", not the result.
What clinics use WhatsApp for
1. Appointment confirmations and reminders
The single biggest win. A confirmation when the slot is booked and a reminder the day before cut no-shows and the calls asking "what time was my appointment?". Add buttons such as Confirm and Reschedule, and route anyone who taps Reschedule straight to the front desk.
2. "Your report is ready"
Diagnostic centres and hospitals send a short update when a report is ready to collect or download from their own portal. Keep the report itself out of the WhatsApp message (more on why below).
3. Answering enquiries, including after hours
Most messages are the same questions: timings, fees, which doctor is in today, the address. An automatic reply can answer them instantly, show today's timings, and tell people outside opening hours when someone will call back. Anything else goes to a person.
4. Leads from ads and Google
If you run click-to-WhatsApp ads, every chat that starts from the ad is free for 72 hours, and an automated first reply can ask what the patient needs before handing over to staff. New enquiries can be copied to a Google Sheet so nothing gets lost.
5. Follow-ups and recall visits
"It's been a year since your last eye check-up" or "Your follow-up visit is due" brings patients back. These are scheduled messages to patients who agreed to hear from you.
6. Payments
Send a payment link for a consultation or a package before the visit, through your own payment gateway account.
7. One number for the whole front desk
With a shared inbox, several people answer from the clinic's one WhatsApp number, each chat is assigned to someone, and internal notes stay inside the team. When a patient calls on WhatsApp, the call can ring whoever is free at the desk.
What it costs in India
Meta charges per delivered message, by type. These are its rates for Indian (+91) numbers, effective 1 October 2026:
| Message | Clinic example | Meta's rate (before GST) | With 18% GST |
|---|---|---|---|
| Utility | Appointment confirmation, reminder, report ready | ₹0.115 | ₹0.136 |
| Authentication | OTP to log in to your patient portal | ₹0.115 | ₹0.136 |
| Marketing | Health camp, check-up package offer, recall visit | ₹0.8631 | ₹1.018 |
| Service | Your replies within 24 hours of a patient's message | First 1,000 a month free, then ₹0.115 | ₹0.136 |
Messages patients send you are always free.
A worked example. A clinic seeing 30 patients a day, 26 days a month, sends a confirmation and a reminder for each of its 780 appointments, 300 "report ready" updates, one check-up camp announcement to 2,000 patients, and 2,500 replies:
| What | Messages | Cost before GST |
|---|---|---|
| Confirmations, reminders and report updates (utility) | 1,860 | ₹213.90 |
| Check-up camp (marketing) | 2,000 | ₹1,726.20 |
| Replies (1,000 free, 1,500 charged) | 2,500 | ₹172.50 |
| Paid to Meta | ₹2,112.60 | |
| Mywa subscription | ₹999 | |
| Total | ₹3,111.60, or ₹3,671.69 with GST |
That's under ₹5 per appointment for everything, campaign included. Notice where the money goes: the one marketing campaign costs eight times more than all the reminders together. Plug in your own numbers in the WhatsApp API pricing calculator for India.
The rules for healthcare messages
This isn't legal advice, but these are the rules every clinic should know before starting.
Meta's WhatsApp Business Messaging Policy:
- Opt-in first. You can only message people who gave you their number and agreed to hear from you on WhatsApp. Ask at registration: a line on the form, or a checkbox at the desk.
- Respect opt-outs, on or off WhatsApp. If a patient says stop, stop.
- Health information needs care. Meta's policy says not to use WhatsApp for telemedicine or to send or request health information where the law requires systems with higher protection. The safe pattern: send a notification ("your report is ready, collect it or download it from our portal") and keep diagnoses and results in your own system.
- No targeting by condition. The policy bars discriminating on the basis of a medical condition, so don't build campaigns around patients' diagnoses.
India's Telemedicine Practice Guidelines (2020) name WhatsApp as one of the channels doctors may use for teleconsultation, with conditions: the doctor's registration number on prescriptions and on electronic messages, the patient's identity verified, and consent recorded. Prescribing over chat has limits that depend on the medicine. If doctors consult on WhatsApp, read the guidelines first. For most clinics, it's simpler to keep WhatsApp for the front desk.
The Digital Personal Data Protection Act, 2023. Its rules were notified in November 2025, and most obligations (clear notices, consent you can prove, security safeguards and breach reporting) apply 18 months later, from around May 2027. Patient phone numbers and appointment details are personal data, so collecting proper consent now saves rework later.
How to set it up with Mywa
- Connect your clinic's number through Meta's Embedded Signup. If the number is already on the WhatsApp Business app, it can stay on the app with coexistence. See how to connect a number to the WhatsApp Business API.
- Create your templates: appointment confirmation, reminder, report ready and follow-up. Submit them to Meta from inside Mywa. Reminders and updates go in as utility; offers and camps as marketing. Not sure which? Try the template checker.
- Send reminders automatically. If your booking or hospital software can send a webhook, or you use a tool like Zapier or Make, an automation in Mywa sends the confirmation the moment a slot is booked. No software? Import the next day's appointments as a contact list each evening and schedule the reminder.
- Set up the front desk inbox. Add up to 5 team members, set your opening hours, and turn on an automatic reply for common questions and for messages outside hours.
- Add buttons and routing. Confirm and Reschedule buttons on the reminder; anyone who taps Reschedule is assigned to the front desk with a notification.
Mywa is ₹999 a month (₹9,999 a year) plus GST, with every feature and 5 team members included. Meta bills the message charges directly to your own WhatsApp Business Account at its official rates, with no markup. See Mywa pricing.
The bottom line
WhatsApp is where patients already are. Used for the front desk (reminders, reports, questions and payments), it cuts no-shows and phone calls for a few rupees per patient. Ask for consent at registration, keep health details out of messages, and save marketing messages for the occasional camp or recall reminder, because they cost far more than everything else.
